CheckUP Health in Focus annual survey 2018

CheckUP's annual Health in Focus survey is designed to capture a snapshot of the issues facing patients and health providers in Queensland.

The annual survey is in its 10th year and has evolved significantly since it begun. During the past decade over 11,000 people have taken the time to respond to the survey. The aim of the survey is to investigate issues faced by primary health practitioners and the general public in Queensland. The survey focuses on a range of topics including workforce models, care coordination, preventative health, digital health and interface between different sectors. 

If you choose to complete the survey, you will have the option to provide your name and phone number for a chance to win one of three Apple Watches, an iPad or a Fitbit.

The survey will close on 7 December 2018.
